We adopted an adult cat a little over a month ago. She uses her litter box fine, and we remove any clumps or debris at least once or twice a day for her, but how frequently should we change the entire litter and wash out the box?

At least once a day scope clumped litter and replace it compeletly once every week to a week and a half to keep your home from smelling like a big litter box. And to keep bacteria to a minimum.

I clean out the clumps every 3rd day or everyother day and it helps and then in the middle I change my cats litter once a month if needed.
I scoop several times daily, and thoroughly clean the litter box whenever it looks like it needs to be done. It can vary.
You're doing fine with removing 1 - 2 times a day. Change the entire litter at least once a week, including a good scrub of the box with ho water. If you can, do it twice.
I hate clumping litter and as a breeder I certainly don't recommend it. You remove the clumps but the bacteria stays behind and builds up.

IF I was to consider using it I would still dump it once per week. I would also have a second box so that I could change it out and let the dirty pan sit in disinfectant for at least 10 minutes.
Remove the clumps every day, probably at the end of the day so that you dont have to do it again. I change the whole entire litter box about once every 6 months. Wash the box thoroughly but dont use perfumed soap because that will give the cat an unfamiliar smell and might cause her to pee somewhere else (on like your carpet or something)

Sounds like you're doing fine with cleaning out the clump. Usually you want to clean the litter box out weekly or bi-weekly, depending on how much waste your cat is producing.
Make sure you wash out the box in between changes. You can also buy litter bags or use a small trash bag to line the box with so that it's easier to clean. That way, when you have to remove the litter itself, all you have to do is lift the bag and throw it away.

If you're scooping daily, a monthly washout is fine.
about 2 a week ... 3 and 1/2 days .. depending ... you dont need to wash out but 1once a week.
I scoop once a day and clean the whole box once a week. I wash it with bleach for desinfection, flush it with clean water so the cats don't smell the bleach.
I scoop the box every other day, and usually empty the whole thing and clean/disinfect about once a month. The whole point of scoopable cat litter is that you don't have to clean the box as often. However, there are times when I do it more often depending on how much the cats are going. Trust me... your cat will let you know if her litter is unacceptable!!
